HEALTH CARD
Apply for a Health Card
All new Saskatchewan residents must register themselves and their dependents for a Saskatchewan Health Card in order to receive health benefits.
New Saskatchewan residents are people who have relocated to Saskatchewan from another Canadian province/territory, or from outside Canada, such as foreign nationals, international students and returning Canadians.
You may complete the application for yourself, your spouse/partner and all dependents under 18 years of age if they are living with you in Saskatchewan.
How to apply for a Health Card:
- Find out if you are eligible to apply for a Health Card for yourself, or on someone’s behalf.
- Make sure you have the necessary supporting documents to accompany your application.
- Electronic copies of your documents may be obtained by scanning or by taking digital pictures of your documents. Please ensure that all of the information on your documents is readable.
- Start your online application
You can also choose to print and complete a Health Card Application form. Mail, fax or email your completed form and supporting documentation.
Health Card applications are usually processed within six to eight weeks after we receive your application. This does not include mailing time.
